THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This Notice applies to Luma Sonara Concierge PLLC and Drava Infusions PLLC, and to the workforce and business associates who support them.

How we may use and disclose your health information

Treatment

We use and disclose your health information to provide, coordinate, and manage your care, including sharing it with other providers involved in your treatment.

Payment

We use and disclose your health information to bill and collect for services, and to administer memberships.

Health care operations

We use and disclose your health information for quality assessment, staff review, training, compliance, business planning, and general administration.

Appointment reminders and communications

We may contact you about appointments, treatment options, and health-related services. If you ask us to communicate with you by text or email, we will do so, and we will tell you that those channels are not secure.

Other uses and disclosures permitted or required by law

Uses requiring your written authorization

Most uses and disclosures of psychotherapy notes, uses for marketing purposes, and any sale of protected health information require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.

Our duties

We are required by law to maintain the privacy of your health information, to give you this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. We reserve the right to change this Notice and to make the revised Notice effective for information we already hold as well as information we receive in the future. Current copies will be posted at our location and on our website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us using the contact details below, or with the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
